By registering for programs at any of the Carnegie Museums, I agree to the following (if registering a minor for the Program, I agree on behalf of myself and my child, and if registering on behalf of an organization, I agree on behalf of myself, the Organization, and its participants):
I am 18 years of age or older. I am registering myself or I am the parent or legal guardian of a child I am registering, and I consent to my child participating in the Program. Or, if registering on behalf of an organization, I am authorized by the Organization to register the Organization and its participants for the Program, the Organization for itself and for its participants consents to its/their participation in the Program, the Organization agrees to engage a third-party vendor to administer and host the Program unless otherwise indicated in the Program Description, and I represent for myself and the Organization that the Organization has obtained from the participants any and all necessary consents, agreements, and releases required by this registration or any applicable laws or regulations.
I/we agree to abide by the Program Terms and Carnegie Museums’ Privacy Policy and other applicable policies as amended from time to time. In addition, if Carnegie Museums has engaged a third-party vendor to administer or host the Program, I/we agree to abide by the terms and conditions of that third-party vendor identified in the Program Description. I/we for myself, for the minor I am registering, if any, and my/our heirs, next of kin, executors, and insurers; and if registering on behalf of an organization, the Organization for itself, its officers, board of directors, and insurers, and for its participants and their parents and legal guardians; hereby irrevocably and unconditionally release Carnegie Museums and those acting with or for Carnegie Museums from any and all liability relating to participation in the Program and/or use of Program content.
